Perimenopause Therapy & Counseling in Arizona: Show Low, the White Mountains, and Online

Support for perimenopause anxiety, insomnia, mood swings, low libido, and the strain midlife changes put on your relationship.

If you're in your late 30s, 40s, or early 50s and suddenly can't sleep, feel anxious for no reason, snap at the people you love, or have lost interest in sex, you may be in perimenopause. Perimenopause is the transition before menopause, and it can last anywhere from a few years to a decade. For many women, the emotional and relational symptoms hit harder than the physical ones, and they're the ones nobody warned them about.

I'm Sarah Simpson, LCSW, a Certified Sex Therapist (AASECT) completing my PhD in Clinical Sexology. I offer perimenopause therapy for women, partners, and couples in person in Show Low, Arizona, and online throughout Arizona and Missouri. My work focuses on the nervous system, the relationship, and sexual health during midlife.

What is perimenopause?

Perimenopause is the stage when estrogen and progesterone levels start to rise and fall unpredictably as the body moves toward menopause. Menopause is officially reached after 12 months without a period. Perimenopause can begin as early as the mid-30s, though most women notice changes in their 40s.

Because hormones affect sleep, mood, stress response, and desire, perimenopause can feel like your body and brain changed overnight. Many women are told it's "just stress," "just aging," or "all in your head." It isn't. These changes are real, and support helps.

Common emotional symptoms of perimenopause

  • Perimenopause anxiety: sudden worry, dread, racing heart, or panic attacks you never had before

  • Insomnia and night waking: trouble falling asleep, waking at 2 or 3 a.m., night sweats that disrupt rest

  • Mood swings and irritability: perimenopause rage, short temper, crying more easily

  • Low mood or depression: feeling flat, unmotivated, or not like yourself

  • Brain fog: forgetfulness, trouble focusing, losing words mid-sentence

  • Feeling overwhelmed: less capacity to handle stress at work and at home

  • Identity shifts: grief, questions about purpose, or feeling invisible in midlife

Perimenopause and your relationship

Perimenopause doesn't only happen to one person. It happens to the whole relationship. When sleep falls apart and your nervous system stays on high alert, there's less patience, less playfulness, and less room for connection. Partners often feel confused, shut out, or rejected, and couples can start to feel like roommates.

Common relationship struggles during perimenopause include:

  • More frequent arguments and irritability

  • Feeling misunderstood or unsupported by your partner

  • Emotional distance and loneliness

  • Partners taking mood changes personally

  • Mismatched libido and tension around sex

Perimenopause couples counseling helps both partners understand what's happening, stop blaming each other, and work as a team through this season.

Low libido and sex during perimenopause

Low libido in perimenopause is one of the most common concerns I hear. Shifting hormones, poor sleep, anxiety, vaginal dryness, and painful sex can all make desire harder to reach. One partner may want sex more than the other, leading to a painful cycle of pursuing and avoiding.

As a Certified Sex Therapist, I help women and couples understand how perimenopause affects desire and arousal, reduce pressure around sex, and rebuild intimacy that feels good. Desire usually isn't gone. It's harder to access when the body is exhausted and stressed. I'm glad to coordinate with your OB-GYN, menopause practitioner, or pelvic floor physical therapist.

How therapy helps with perimenopause

Therapy for perimenopause doesn't replace medical care. It works alongside it. Hormone therapy and other medical treatments are decisions to make with your doctor. Therapy helps with the parts medicine often doesn't touch: your nervous system, your emotions, your relationships, and your sense of self.

In perimenopause therapy, we may work on:

  • Calming anxiety with body-based, somatic tools you can use anywhere

  • Improving sleep by settling your nervous system before bed and after night waking

  • Managing mood swings and creating space between a trigger and your reaction

  • Rebuilding connection with your partner through couples therapy

  • Addressing low libido and painful sex through sex therapy

  • Navigating midlife transitions such as empty nest, aging parents, divorce, or career change

I draw on Emotionally Focused Therapy, Somatic Experiencing, the Gottman Method, and sex therapy, so we work with both your mind and your body.

Who I work with

  • Women in perimenopause who want support for anxiety, sleep, mood, and identity

  • Couples whose relationship is strained by perimenopause and midlife changes

  • Partners who want to understand and support someone going through perimenopause

All identities and relationship structures are welcome.

Frequently asked questions

Can perimenopause cause anxiety? Yes. Fluctuating estrogen and progesterone affect brain chemistry and the stress response, and many women experience new or worsening anxiety, panic attacks, or a constant sense of dread during perimenopause.

Can perimenopause cause insomnia? Yes. Hormonal shifts, night sweats, and an activated nervous system commonly cause trouble falling asleep and early-morning waking.

Can perimenopause affect my marriage? Often, yes. Mood changes, poor sleep, and low libido can create distance and conflict. Couples counseling helps partners understand what's happening and reconnect.

Why has my sex drive dropped in perimenopause? Hormonal changes, exhaustion, stress, and physical discomfort can all lower desire. Sex therapy can help you understand your changing desire and find intimacy that works for you now.

Do I need a diagnosis to start therapy? No. If you're noticing changes in your mood, sleep, anxiety, or relationship in midlife, that's reason enough to reach out.

Do you prescribe hormones? No. I'm a therapist, not a medical provider. I work alongside your doctor or menopause specialist.

Can my partner come to sessions? Yes. I see women individually, couples together, and partners on their own.

How much does perimenopause therapy cost? Individual sessions are $175 for 50 minutes, and couples sessions are $185 for 60 minutes. I'm cash-pay and provide superbills for possible insurance reimbursement.
